The template follows the structure a violence risk report needs to survive cross-examination. It opens with identifying information and the referral question stated verbatim, because the scope of a risk opinion is bounded by the question asked. Notification and the limits of confidentiality come next, documented rather than assumed, followed by a numbered sources-of-information section separating records reviewed, interviews conducted, collateral contacts, and instruments administered.
History is scoped to what bears on risk. A dedicated history-of-violence section documents the nature, frequency, recency, and severity of past violent and threatening behavior with a source for every item, then analyzes the pattern: escalation, triggers, and likely targets. Behavioral observations and mental status follow, then testing, with response style and symptom validity addressed before any substantive result and each instrument named alongside its intended use and known limits.
The analytic core is the risk formulation. Rather than a bare high or low label, the template separates static, dynamic, and protective factors, then builds a scenario formulation: plausible violence scenarios, likely warning signs, and the conditions under which risk rises or falls. The communication-of-risk section keeps the opinion conditional and tied to identifiable circumstances, and the risk management section translates the dynamic factors into monitoring and intervention recommendations where the referral asks for them. An attestation block closes the report.

Why scenario planning instead of a single risk score?
Because a single global label communicates less than a conditional formulation. The template prompts for plausible scenarios, likely targets, warning signs, and the conditions that raise or lower risk. That is both more useful to the referring party and more defensible on the stand than a number presented as if it were precise.
